If you plan to leave your minor kids at home with the grandparents or other relatives, it is always best to prepare for possible emergencies. By filling out this form, you are giving the temporary caretaker permission to seek emergency care for your children. If there is an emergency, the grandparent or other relative (or friend) can show the consent form to the doctor or other medical personnel. It's a good idea to copy your health insurance cards and attach the copies to this form.
Your child's school may also have a specific form for you to use. Be sure to ask before leaving town!
